File Proof of Debt

If you are a creditor of one of the insolvencies currently adminstered by HFK please provide details of your claim on the following proof of debt form. Completion of this form will enable you to receive 6 monthly Liquidator reports and will ensure that you are liable to partake in any distributions made to creditors.

Please print the pdf file and return it to PO Box 39-100, Christchurch 8545. Alternatively this can be scanned and emailed to insolvency@hfk.co.nz
